These tax forms are handled by the departments most closely associated with their content. In accordance with IRS guidelines, they are mailed on or before January 31st of each year.
For questions regarding a 1099-Misc form you received from the University, please contact the University Financial Helpline at 612-624-1617.
For questions regarding a 1099-R form you received from the University, please contact the Human Resources Management System at 612-625-2016.
If you are a University Department which received a 1099-Misc form from outside of the University, please send it via campus mail to the Tax Management Office - Suite 111 University Office Plaza, Mail Code 2715. Please be sure it include your name & contact information as well.
1098T Forms, Statements of Tuition, are handled by One Stop Student Services. They have developed a very informative FAQ list which is hosted on our site. If after consulting the FAQ on 1098T forms you still have questions, please consult One Stop directly.
W2 forms: If you have not received your W2 form within a reasonable time period AFTER January 31st, you may access the Employee Self Service web page to request a duplicate. If you have questions regarding your W2, please consult the Payroll Department directly. 612-625-2016 or payroll@umn.edu.
1099 Forms - FROM the University

1099 Forms - TO the University
The University of Minnesota is exempt from most 1099 reporting, however many companies still send out 1099s. The Tax Management Office collects and retains these documents, so if you receive them at another University address, please forward them to us via campus mail. Be sure to include a note with your name and contact info, so we know where the documents were routed from. Phone: 612-624-1053
